CONSTITUTION & BYLAWS / MANUALS*

NOTE: ALL REPORTS are requested via EMAIL. If you choose to mail in a report, please email appropriate Officer for address. For assistance, contact INFO@PurpleHeartAuxiliary.org. Home addresses will not be posted online.

If you have not received a confirmation that your report has been received, please follow-up with he intended receiver(s).

Use a
supplemental sheet if you need additional space to complete your report. 

DUE MAY 1st of each year

SEND FORM TO: Your Department Chairperson (if applicable), Unit Secretary and National Chair.

SEND FORM TO: Your Department Chairperson (if applicable), Unit Secretary, National Membership Officer, and National Chaplain.

SEND FORM TO: Your Department Chairperson (if applicable), Unit Secretary and National Chair.

IMMEDIATELY UPON INSTALLATION, SEND FORM TO: National Secretary, applicable Region President or Region Liaison, and your Dept. Secretary where a Department exists.
NOTE: Member changes in address, email, etc. should be provided to the National Membership Officer, and National Secretary.

DUE MAY 15th of each year

SEND FORM TO: National Chaplain, and National Membership Officer, and Department Secretary.

SEND FORM TO: National Chairperson, and a copy to the Department Secretary.

Department, Region Presidents, or the National Inspector (usually Sr. Vice President) must inspect Units annually. This Inspection report is due on May 15th of each year. A copy of the report MUST be sent to the National Secretary. Note: Departments are not inspected. Only Units, regardless of if they have a Department or not, are inspected.

Instructions to the Inspector:
•No member of the Unit may inspect their own Unit.

•Complete this form for ALL Units within your Department, except your own Unit.

•An Inspector’s own Unit may be inspected by a Region President, PNP, PDP, or other Nat'l. Officer

•Send a copy of this report to the National Inspector

•Completion of this form may be done by phone or in person with the Unit President or designated representative.

SEND FORM TO: Your National VAVS Chairperson Linda Varejcka at JFVarejcka@Windstream.net, a copy to you Unit Secretary, and your Department Secretary if applicable.

Officer/Chairman Reports

DUE by June 30th

Remember: In order for members to vote during the National Convention, the Unit/Department MUST have sent in the Installation Report to the National Secretary by May 31

•Send a copy of this report to the National Secretary on or before June 30th.

DUE from July 1 - October 1st

UNITS and DEPARTMENTS MUST ATTACH CONFIRMATION OF ACCEPTED & APPROVED 990 FILLING from the IRS website.

UNITS: Send one copy to MOPHA National Treasurer, your Department Treasurer, and keep one for your files.

DEPARTMENTS: Send to MOPHA National Treasurer and keep a copy for your files. This form, with 990 filling confirmation, MUST BE SUBMITTED to the National Treasurer by October 1st in order to remain in good standing and to receive your Life Member Rebate (if applicable).

Provide copies to National Membership Officer, National Secretary and Department Secretary if applicable, no earlier than July 1, and no later than October 1.

The Life Member Verification Report is a MANDATORY report to be considered in good standing.
